To say Happy birthday in Hebrew, it is quit easy, the phrase is a 3-word sentence, since birthday is actually two words in Hebrew. Birth and day.
So here is the phrase in details
יום הולדת שמח
yom huledet sameach
יום = yom means “day”
הולדת = huledet means “birth”
שמח = sameach means “happy”
As you notice, in Hebrew the adjective comes after the noun, unlike in English or German or other Latin-oriented languages. So we literally say “birthday happy”.
